Walk-in for Pfizer adult, Pfizer Pediatric and Moderna are available Monday to Friday 10am-5pm and Saturday 10am-2pm.

Checklist Before Your Vaccination

1. Read the Prevaccination Checklist for COVID-19 Vaccines.
2. Complete the COVID-19 Consent Form. (MODERNA, JANSSEN, PFIZER, PFIZER PEDIATRIC)
3. Read the fact sheet for each vaccine:
   • PFIZER
   • JANSSEN
   • MODERNA
   • PFIZER PEDIATRIC
4. Read the Wellness Family Pharmacy notice of privacy
5. Make sure you have a photocopy of the following:
   • Driver’s license/State ID
   • Prescription insurance card
   • Medicare A & B card (For adults 65+)

Post-Vaccine Resources

1. Sign-up with v-sage for check up after your shot at v-safe (cdc.gov)
2. Report side effects to help everyone learn more about the vaccine. You can report post vaccination side effects at Vaccine Adverse Event Reporting System (VAERS) (hhs.gov)
